Joint Replacements; Primary Prevention?




A Hypothesis Concerning the Primary Prevention of
Joint Disease, Disc Syndrome, Spinal Stenosis and Osteoarthritis

It is a well known fact to medical students and physicians that cartilages and spinal discs are "avascular" indicating that they do not have visible blood vessels. This does not mean that these cells can survive without blood circulation.
It is a well known fact to medical students and physicians that an average adult man's cardiac output(minute volume=stroke volume times heart rate) is about 5000 ccs at rest and that an average man can increase
the cardiac output five folds to 25000 ccs
by the maximal aerobic exercise.
It is a well known fact to medical students and physicians that every single cell of our body has to be replaced on an
average every 7 years.(see comment below)
Our cells need blood circulation to replace our cells.
Here is the problem for joint cartilages and spine discs in that they do not receive enough blood circulation to replace the cells, not as much as the rest of the body cells.
As a consequence, after many years of usuage, joint cartilages and discs wear out or degenerate and gradually vanish, resulting in so called "bone and bone without cartilages" description by the orthopedic surgeons before they recommend hip or knee replacements.
Almost every PGA golfer has back trouble interestingly except Tiger Woods who is known to do the high level aerobic exercises.

Hypothesis:
You can prevent joint replacements and laminectomies or osteoarthritis by doing the high level aerobic exercises good enough to increase the cardiac output 4 or 5 folds which will deliver blood to the cartilage and disc cells so that they can be replaced fresh.
In other words, the health of the joints or cartilages and
discs depends upon how much and how fast one is wearing
them out relative to how much and how fast one's body is
generating the new cartilage and disc cells which depends
upon how much aerobic, cardiovascular exercise one is doing
everyday.

Observations:
Starting with myself who experienced disc syndrome some ten years ago
and other patients who have suffered joint and disc disease yet still good enough to carry out the high demand of the aerobic exercises on regular basis have demonstrated the expected gradual clearing of the symptoms and diseases.
One has to start from low level to high level in a gradual manner as I have done in the last 10 to 20 years.
Some other observational facts:
People who do endurance swimming for their aerobic
exercises over 10 years appear to be immune from
the complications from osteoarthritis being discussed here.
I have a 91 year old female patient who had been swimming
practically all her life until a couple years ago and
was swimming competitively, winning medals, etc.
She is basically free of any significant osteoarthritis,
free of any joint or back pains in contrast to her
contemporaries.
It makes sense because swimming is known to tax the joints
less so that the swimming exercise wears out less cartilage
and disc cells while producing extra fresh cells due to the
higher cardiac output.
People who jogs 3 to 5 miles or medium distance, not
marathon, appear to be immune as well.
Elderly people full of osteoarthritis and lots of pains who
have been put through water aerobic classes regain
their mobility.
All of these, I believe, have to do with regeneration of
cartilages and discs.

The purpose of this presentation is to help our aging alumni and their spouses in time to prevent the wide spread medical problems with joints and back as they get older.
